Criminal Justice Case Management

 

 

Under contracts with the California Department of Corrections and Rehabilitation, Division of Addiction and Recovery Services, Center Point's Community Services Coordinators provide an array of assessment, placement, and case management services throughout California. Community Services Coordinators assess participants of the In-Prison Substance Abuse Programs evaluate the treatment and service needs of recent parolees manage the development of community-based aftercare plans for parolees, which complement their in-custody or previous treatment experience match parolee service needs to the appropriate levels of treatment and provide transportation to and from community treatment programs. In partnership and through coordinated efforts with parole agents and providers, Community Services Coordinators conduct monthly follow-up to ensure treatment compliance and monitor client progress in treatment. In addition, the Community Services Coordinators evaluate the continuing care needs of parolees in order to develop referral pathways for employment, housing, and other aftercare support.

 

Through the SASCA Region II and BASN contracts, Community Services Coordinators provide case management services to incarcerated and recently paroled men and women who have successfully completed California Department of Corrections and Rehabilitation in-prison substance abuse treatment programs and who are paroling to one of the nineteen coastal counties, from the Northern California border to Ventura County in the south.

 

SASCA Community Services Coordinators conduct outreach and engagement activities within the prisons and initiate contact with clients six months prior to parole in order to facilitate parolee placement in community-based treatment for parolees who have completed the in-prison programs. The Community Services Coordinators assist the in-prison participant with the transition to community-based treatment and facilitate client-matching protocols that ensure placement in the most appropriate modality and level of care.

 

SASCA Region II provides follow-up and other supportive services to help ensure the success of clients during community re-entry. Securing funding, confirming eligibility, providing transportation, and coordinating placement are the integral components of the SASCA follow-up services. Community Services Coordinators provide referrals for housing and employment services gender responsive case management services and referrals to social support services and other organizations that help to sustain recovery efforts.

 

Eligibility for SASCA and BASN services includes individuals who have participated in and successfully completed a sanctioned substance abuse program in one of California&rsquos State Prisons state prison inmates who meet the criteria for Drug Treatment Furlough and parolees referred by their Parole Agent.

 

SASCA funding and services are available for up to 180 days of residential and/or outpatient treatment and transitional housing. For many participants, the successful completion of community-based treatment results in early termination of parole.
